Are you and your
contestants members? If not, you must join by
March 1.
MEMBERSHIP WILL BE VERIFIED AND IS A REQUIREMENT TO COMPETE.

PAYMENT:
Payment is required within 30 days unless a PO number is submitted. The PO
number initiates a grace period for up to 60 days. Please review our
payment policy
for further information.

Statement of Assurance:
Every Advisor must complete this form to verify that you have collected
completed Medical & Personal Liability forms from your students. The
Statement of Assurance is only completed once per school year. |

VOTING DELEGATES:
Voting Delegates serve to elect the State Officer Team and
approve Constitutional changes. Each section is entitled to Delegates based
on membership. |

State Officer Candidate APPLICATION:
Each Candidate must submit their application by
April 4, 2008. |

WYNDHAM Housing Form
All schools, that are utilizing a hotel, are expected to stay at the
Wyndham. Submit the Housing Form (Word,
PDF) to the hotel by March 15, 2008. |

AZ Diamondback Tickets:
Tickets are included in your conference
registration if you're staying at the Wyndham Downtown Phoenix Hotel.
Otherwise, you can purchase them for $10 each
here. |
